The Exceptional Family Member Program (EFMP) provides assistance to military families with special needs. An exceptional family member can be a child, spouse, or a dependent parent who requires medical or educational services due to a physical, intellectual, or emotional need. Some examples of a special need would be allergies, asthma, autism, attention deficit disorder, cerebral palsy, dyslexia, depression, diabetes, and lupus, and other conditions that may not be listed. Enrollment in the EFMP is mandatory and structured to assign Marines to installations where their exceptional family member can receive needed care. The EFMP Manager provides assistance with enrollment, medical and educational resources, advocacy, workshops, respite care, and family support groups. Exceptional family member assistance is provided before, during, and after transitioning to a new duty station. The EFMP Case Manager provides assistance with enrollment, medical and educational resources, advocacy, respite care and more. The EFMP Education, Training and Outreach Specialist provides assistance with Command and family educational workshops, training, events, support groups, and resources. - DD Form 2792 EFMP Medical Summary. This form is completed by both the sponsor or parent and the qualified medical provider. Please make sure that the demographic information on page 2 (blocks 1a through 6c) are completed. Please make sure the sign and date the form in block
6 (a-c). Pages 3 through 7 are completed by a qualified medical provider.

– Special Education/Early Intervention Summary. This form is completed by the parent and/or sponsor and the school representative. This form must be completed on all children, ages 5-21, enrolled in grades K-12. The parent and/or sponsor should complete the demographic information on page 2 and blocks 1 and 2 on page 3. The school representative completes the information in blocks 3 through 6 on page 3. If your child is receiving accommodations or services through the school system please bring in a copy of the Individualized Education Plan, 504 Plan, or Student Plan specifying the accommodations and/or services.
- Authorization to Release and Consent to Exchange Information. In order to provide services the sponsor and/or spouse must sign a release form for minors. Anyone over the age of 18 has to sign their own Authorization to Release and Consent form. A signed copy of this form allows the local EFMP office to provide you with assistance, referrals, and respite care.
- Declination of Family Case Management. A sponsor and/or family member may also decline family case management services provided by the installation EFMP office. If you elect to sign the Declination of Services, the local EFMP office will have no contact with you or your family. Contact will be from HQMC only.
